What Information Do I Need to Provide When Booking?

The information required depends on the type of session you are booking.

One-on-One Sessions: you must provide the required information and complete all compulsory forms and preparation before the Session. This includes the required Intake Form and preparation instructions provided for the Session.

Group Sessions: the Intake Form is optional. You may use it to provide questions or prayer requests for consideration. Completing the form is not required for attendance.

Make sure that the information you provide during booking is accurate and that you provide any required information or forms by the requested time.
